The NTSB has released its preliminary report in a plane crash that killed a pilot and injured two others last month in Palo Alto.

The report says the pilot W. John Spencer had trouble finding the airport, despite having flown to it several times in the past.

Spencer was providing a free flight for a woman and her daughter through the nonprofit Angel Flight West organization when the plane crashed on Sept. 4.

Witnesses say they saw the plane bouncing on the runway during an attempt to land, then lift off again and bank steeply to the left before descending to the ground.

Spencer had been volunteering for the nonprofit since 2014.

He had donated more than 125 flights to families and had flown to the airport three other times.
